Family Forms

The family forms below are designed for use by everyone, but are especially helpful to individuals who wish to represent themselves (pro se) in court matters related to family law.

If you have a question about the content of the forms, or you can't find the one you are looking for, please contact the Self-Help Center nearest you.

Not all motions or other documents that would be required in any given individual's specific case will be found here. What you will find below are forms that the Florida Supreme Court has approved for public consumption, based upon opinions the Court has issued.

The forms below are listed chronologically according to the section of Florida Rules of Court that pertains to family law, which is section 12. Therefore all the following forms have a number 12 prefix.

The forms have been created in a WordPerfect 8 format. Beside each form name, you will have an option of opening the form three different ways. Normally you will want to open them through the "PDF" option. Only choose "ZIP" or "EXE" if you understand these two program options and you think your computer can handle these two operations.

The forms can be completed on-line if you choose the PDF version. If you choose the ZIP or EXE versions, the forms can be saved to your hard-drive and then filled out.

Instructions for Downloading PDF Files

Be certain you have Adobe Acrobat 4.0 or higher installed on your computer. If not, download Adobe Acrobat Reader now.

Adobe Acrobat Reader is free and will allow you to complete forms on-line and print them out but not save them. After downloading the software, return to this page to view the PDF files. If you are still having problems, reboot your computer and try viewing the PDF files again.

• To begin typing in the form, use your mouse to click in the field and begin typing. To continue to navigate through the form, use the mouse to click in other fields or use the Tab key.
• Do not use the Return key inside the form for navigation between the fields.
• To scroll through the document, use the Scroll Bar or click on the page advance icons (first page, next page, previous page, last page)on the menu bar at the top of the document to advance page by page through the document.
• To make a check-mark, use your mouse and left-click in the area.
• Some fields (such as signature fields, date fields and notary fields) are intentionally left blank. You will need to print the completed form and have the respective individual sign and date the appropriate fields.
• If you are using the Reader (and not the full version of Acrobat) you can not save the form after you have filled it out.

Instructions for Downloading Forms - EXE

• Click on the "EXE".
• Choose the option to "run this program from its current location".
• Click "Yes" when asked if you wish to install and run the file. This option says that the "autheticode signature not found" but you will need to click "yes" to proceed.
• A pop-up message will appear explaining that the file is being placed on your C:drive in a folder called "forms".
• Open your word processor (MS Word or Corel WordPerfect - you cannot open the file in MS Works) and click on "file" and then "open" and then browse to your C: drive and to the "forms" folder and double-click on the wordperfect file.  MS Word will convert the file if the converter is installed.  If it isn't, you will need to do a custom install using the installation CD. If you are using MS Word, make sure the Files of Type option is set to "all files".

Instructions for Downloading Forms ZIP

• Click the "ZIP".
  
• Click on the "Extract" icon at the top.
  
• Browse to the drive/folder on the right side of the box where you want to save the file and click on the "extract" button.
  
• Close out of the win-zip application by clicking on the "x" in the upper right-hand corner.
  
• Follow the procedure in 2e above for opening the file in your word processor.
• Open your word processor (MS Word or Corel WordPerfect - you cannot open the file in MS Works) and click on "file" and then "open" and then browse to your C: drive and to the "forms" folder and double-click on the wordperfect file.  MS Word will convert the file if the converter is installed.  If it isn't, you will need to do a custom install using the installation CD.
